What you’ll need. A draft form and a list of essential questions.

  1. Match the field to the answer

    Use email, phone, number, date, address, or name fields for structured information. Use choices when you need consistent answers and a text area for longer responses.

  2. Add supporting information

    Use uploads, signature fields, photo capture, ratings, tables, and terms fields when they are relevant to the task.

  3. Group related questions

    Arrange fields in rows or reusable containers. Add headings and dividers, and split a long process into pages.

  4. Make the page readable

    Use themes, images, and landing-page sections to provide context. Check the result on a small screen and avoid asking for information twice.
